Once the relisting is done you will be issued a share holder reference number (SRN). Once this has been issued you can do a chess transfer request using either a form or online. If you use Westpac like myself My Account – My Portfolio – Holdings & Accounts then select Transfers if not ask your broker for the navigation path. The form required is Chess sponsorship and Issuer Sponsored Transfer.
