Drilling accuracy

I am not technically wise regarding drilling and seismic stuff so just interested to learn from the last drill in NZ. From those knowledgable ones here I wonder if you could suggest why we missed the primary target? Would it be because the 3D surveys always contain a degree of inaccuracy or is it not possible to direct the drill bit totally accurately - or perhaps a bit of both?? Or would be attributable to other factors??
