you mentioned the possability of heart valve failure potentually cropping up, IMU did run trials on beagle dogs for this as they are really suseptable to this issue, no such issues were found. But you need to find your own level of confidence, perhaps you need to wait untill maybe 20 or (how many people do you want to see?) so safely through the trial?

It is well accepted that synthetic T cell vaccines such as Herceptain are problem-matic in the way that you have outlined, but then thats the whole reason for IMU's technology it is not synthetic and experts in this field agree on one thing and that is that toxicity is more than likely going to be a rare occurance.
